Thorough History
We make it easy to schedule your first appointment by calling (905) 878-8528 or filling in our online request form here. It is our goal to learn all the details of your medical history to ensure each patient receives the specialized treatment they deserve. Our New Patient form can be downloaded here and completed prior to your appointment so we can learn everything we need to know about your medical history. You can choose to bring it with you, email it to us or even drop it in the mail:

If you have a hectic schedule you can fill in the forms at your appointment. However, we kindly ask that you arrive 15 to 20 minutes early to make sure the form is completed before your consultation begins.
Getting to Know You
A new patient appointment is one of the most important appointments you will make at Milltown Dental. This is the time we get to know you, answer your questions and come up with a dental treatment plan based on your history, dental health, and your own dental goals. Every detail is recorded so we have all the information we need to provide personalized dental care for you.
Meeting Dr. Julie Boudreault
Each new patient is scheduled to see Dr. Julie Boudreault, whose friendly chairside manner will be sure to set you at ease. We want you to be comfortable not only at your first appointment but for all the appointments scheduled for your treatment plan. Dr. Julie encourages new patients to ask as many questions as needed and to express any concerns you might have about your dental health and treatment. You can also discuss concerns you might have about your smile and explore the cosmetic dentistry treatments available.
Comfortable Environment
Our office is designed with your comfort in mind. We want you to enjoy your visits and have added special touches to our treatment rooms such as comfy chairs, neck support, cozy blankets, and TVs. Our play area is designed to help children form happy relationships with our clinic. We help children develop healthy dental habits that will stay with them for life. Nervous patients are offered special considerations including the option to use sedation during their appointments.
For your Convenience
New patients will also be asked to provide any applicable insurance information, so our administrative team can handle all your billing directly. We make it easy to process your claims without the need for paperwork. All dental work on your treatment plan can also be submitted to your insurance company to determine what will be covered. Our team will let you know after your appointment what amount is owed such as your deductible. If you do not have insurance, you can discuss your payment options with our Administrative Team
